LTC Surveyors/Investigators

This individual participates in performing annual surveys or complaint investigations at Skilled Nursing Facilities to ensure compliance with Federal and State regulations. Responsibilities include but are not limited to fully investigating all allegations of abuse, neglect, Quality of Care and Resident Rights and assessing clinical interventions and evaluating patient responses to care plans. This would include patient interviews, staff interviews, chart reviews, observation of care and assessments of the physical plant to assure that patient care is consistent and appropriate. Responsible for preparing written reports of investigative findings and preparing Statement of Deficiencies when necessary. Positions based in Manhattan or Central Islip.

QUALIFICATIONS:

  • Ability to relate effectively with medical and administrative personnel as well as the "public".
  • Ability to travel to facility on-site review assignments.
  • Ability to work independently with minimal supervision.
  • EDUCATION & EXPERIENCE:

  • Licensed as a Registered Professional Nurse in New York State, Registered Dietitian or Certified Social Worker.
  • A minimum of three years experience in a LTC /acute care facility preferably in medicine/surgery or special care units. LTC experience preferred.
